Required Health forms

1. ONLINE MEDICAL INFORMATION FORM

During the registration process, you will complete a Camper Medical Information Form. Our healthcare professionals at camp will have access to this information if your child needs medical attention while attending camp.

2. PROOF OF PHYSICAL

We require proof of a recent physical within one year of your campers session start date. You can upload the physical paperwork from your doctor's office OR take a printed copy of the Health History & Examination Form to your doctor to be completed and signed (click here to download.) These forms can be uploaded online through your Campwise account or emailed to registrar@ligoniercamp.org

Please complete all online forms and upload proof of physical through your Campwise account at least two weeks prior to your campers session.

Next Level / Classic Camp 4: Whitewater rafting

If your camper is attending Next Level, he or she is participating in whitewater rafting on the Youghiogheny River in Ohiopyle, PA. Whitewater rafting is also available during Classic Camp 4 for an additional $85 for campers 12 and older. This requires the completion of the rafting guide company’s waiver. To complete the waiver, click on Whitewater Adventurers, LLC form here. Refer to the Trip Date and Booking ID that corresponds with your session below, and select Lower Yough Guided. Please skip the trip time.
